Thank you for considering adopting a rescue dog.  This form is designed to help us help you find
the right pet for your particular household.  In order to be considered for adoption, you must: 1)
be at least 18 years of age, and 2) understand that this adoption application must be approved.  
Please know the adoption fee BEFORE submitting an application.  Once your application is
approved, you will be sent an acceptance letter but please understand that this does not
guarantee adoption.  It is just the first step and the final decision will be made upon meeting you.
 We reserve the right to refuse an application to any applicant.  If you want to see a dog, please
schedule an appointment.  You will be coming to my home so please DO NOT just show up
unannounced as it is very inconsiderate to me and my family and you will not be provided
service.  Please read and understand the adoption contract before committing to adoption.
